Referrals for Service

For hospital inpatient referrals, Identify if client has a qualifying insurance provider: 1- Molina Medicaid 2- Superior Medicaid 3- Amerigroup Medicaid 4- United 5- Community First If the client has Medicare, in addition to Medicaid, our office can verify eligibility. Referral Steps: 1- Contact one of our Team Leads. San Antonio 210-255-2425, Fax 210-334-0936 Houston 281-783-4511, Fax 281-715-4465 Dallas 972-677-3702, Fax 972-677-3722 Fort Worth 817-856-2232, Fax 817-851-1303 2- Provide basic information so the Team Lead can determine if the client meets criteria for case management services. Name Date of Birth Medicaid Number Diagnosis Hospitalizations in the past 2 years, if available Does client need an outpatient psychiatrist Reason client needs case management Fax the client face sheet to the Team Lead 3- Our Team Lead will schedule a time to meet the client for an assessment and discussion of our services, while still in the hospital if the client is referred while inpatient.  4- If the client agrees to receive services, the Team Lead will coordinate with the referring hospital staff the discharge date and time so a case manager can help them transition from the hospital to their home at discharge. We request discharges no later than 3:00 p.m., if possible, because of the time needed to get prescriptions filled at the pharmacy, discuss immediate needs and plans with the client, and help them settle at home. We understand at times a client may not be able to discharge before 3:00 p.m., and we will still do our best to coordinate a successful transition back home.
